Diabetes Education Program
Due to the protection measures for Covid-19, diabetes education groups are being done on-line. Individual appointments are by phone or personal video conference. If the educator feels it is necessary to see you in person, that will be arranged.

These workshops are available to clients after they have completed our introductory class, "Understanding Diabetes."

The Community Diabetes Education Program of Ottawa (CDEPO) provides individual and group programming to adults with type 2 Diabetes and Prediabetes to help them manage this chronic disease, live healthy and avoid complications. A Registered Nurse (RN) and Registered Dietitian(RD) work as a team to provide care. Our team also includes a Registered Kinesiologist and a Social Support Worker.
Our program has a behavioural focus to promote self-care behaviours that can help people achieve targets for staying healthy. We also work collaboratively with Primary Care Providers and can provide support and recommendations on treatment plans as needed.
We are one of the largest community diabetes education programs in the province, providing services at 9 different locations across Ottawa with teams based out of Community Health Centres and Community Resource Centres.
CDEPO is part of a network of diabetes education programs in Ottawa.
Group education programs - our Group programs are based on the "Wellness Wheel."
Appointments with a Nurse and/or Dietitian who specialize in diabetes
Start people on insulin (in partnership with family physicians, nurse practitioners and specialists)
Follow-up care to help people manage their diabetes
Chiropody/Foot Care Services (referral by healthcare providers)
The Community Diabetes Education Program of Ottawa is funded by the Champlain Local Health Integration Network (Champlain LHIN). It is managed by Centretown Community Health Centre on behalf of the Coalition of Community Health and Resource Centres of Ottawa. We work in partnership with family physicians, nurse practitioners and specialists and with the Champlain Local Health Integration Network.

The Diabetes Central Ottawa – Intake & Referral program is your one access point to community education and support in Ottawa if you are 18 years of age or older and living with prediabetes or type 2 diabetes. It is possible to live well with diabetes through understanding and managing your condition.
For healthcare providers, this program offers one referral form that allows your adult patients living with prediabetes or type 2 diabetes access to effective community diabetes education in Ottawa. For more information, please visit: diabetesottawa.ca
